Patients who have long term health conditions need to be correctly monitored by our practice nurses. Patients who have Asthma, CHD, COPD, Diabetes, Stroke, Hypertension other long term conditions, will receive a reminder from the surgery when it is time for their annual review.
Unless patients come into surgery for regular checks it is not safe for the practice to continue to allow them to order repeat medication, if your medication is due for review you will have to see a GP or a nurse before we can prescribe, if reviews have taken place you will still be able to order repeat medication in your usual way.
Regular reviews of long term conditions pick up any problems with your health before they can become worse, they also enable us to make any changes to medication or referrals that may be necessary in order to maintain your health. We do not hold special clinics for patients with long term conditions you just need to make a 20 min appointment with one of our nurses, at a time that is best for you.
We have extended hours nurse clinics to make it easier for people who work to be able to see a nurse for a review.
